    I hereby grant permission for me/my child to attend and participate in the Alefb multicultural center ("ALEFB") class listed herein and agree to the Terms and conditions set forth herein. I hereby release and discharge ALEFB and its directors, officers, employees, subsidiaries, contractors, and agents from and against all claims and losses of any type arising out of or relating to my child's participation in classes through Alefb.

    I am 18 years of age or older and/or the parent or legal guardian of the above-named child. I have read the Terms and Conditions set forth below, and acknowledge and agree to them.

    ALEFB will have no liability to attendees and hereby disclaims any and all warranties of any kind, express or implied. ALEFB attendees are responsible for their own actions during class. ALEFB reserves the right to remove attendees from any ALEFB class sessions for unacceptable behavior, at ALEFB's or its agents' sole discretion.
